What is You, Me + UC?
You, Me + UC is a resource that is designed to help people living with UC to have open and honest conversations with their doctors, which are vital in shared decision-making. It was created by Pfizer, with input from doctors and patients. It asks questions about areas of your life that your UC may impact. After you finish the questions, you will receive a summary of your answers that we encourage you to share with your doctor. The summary will help your doctor understand what is important to you in your UC management. You, Me + UC is designed to facilitate the conversation and is not to replace those conversations with your doctor.
Will these questions provide an official diagnosis of ulcerative colitis?
No, these questions do not provide any diagnosis. They are only meant to empower you to have an informed discussion about your UC with your health care provider. Only a doctor can diagnose UC.
Who will see my results?
No one will see your results unless you choose to send them. After completing the questions, you will receive a summary of your results that you can keep to yourself or share with your doctor, caregiver, or health care professional. We recommend sharing your summary with your doctor so that you can make the most of your next visit.
Can I share my results with my doctor?
Yes. You will receive a summary of your answers at the end of the questionnaire that you can print or send to your personal email. You may then bring the summary with you to your next visit, or email it directly to your doctor if you choose.
Is my information being stored?
No. We do not store patient information or data from You, Me + UC.
Can I stop halfway and complete it later?
If you exit the questionnaire before completing, your answers will not be saved, as we don’t store the information you provide here.
Will completing this form automatically sign me up for emails?
No. Your email address will not be added to promotional email lists.
How long will the questionnaire take to complete?
These questions are estimated to take between 10 and 15 minutes. However, you are free to take as much time as you need. There is a blend of multiple choice and quantitative questions.
Do I have to answer all the questions?
No, you will always have the option to skip a question if you prefer not to answer. However, we recommend answering every question that you feel comfortable with, as the more information shared can lead to more insight from your doctor regarding your UC.
